In the test order form, please indicate the organism's identity (if known), the original date of specimen collection, and the source of the specimen (e.g. blood, urine, CSF, etc.) from which the bacterium was isolated. If the bacterium's identity is not known, then also request the "Organism Identification, Aerobic" (OIDAER) test. Do not send primary media, mixed cultures, or non-viable organisms. Only send subcultured isolates in pure culture that are visibly growing in the subculture.
