Requisition must be completely filled out with date of procedure, patient's name, clinic number, date of birth or age, clinical history, clinician, procedure location, specimen submitted and any requests for special procedures (i.e. ERICA/PRICA.). Interpretation by pathologist or indicate 'Stain only - no interpretation.' Paraffin block from referring hospital must be labeled with outside hospital name.
