Submit Surgical Pathology Requisition or accompany with an electronically generated test request. Requisition must be completely filled out with date of procedure, patient's name, clinic number, date of birth or age, clinical history, clinician, procedure location, specimen submitted and any requests for special procedures (i.e. stain request). Paraffin blocks from referring hospital must be labeled with outside hospital name. Interpretation by pathologist or indicate 'Stain only - no interpretation.'
