Pathology Consult Requisition

Each section on the Pathology Consult Requisition must be filled out, including:

  • Patient Identifiers
  • Insurance/Billing
  • Clinical Information
  • Ordering Clinician Contact
  • Specimen Details

Supporting Documentation

  • Patient’s pathology report (required)
    • Note: A working copy is sufficient.

Slides & Blocks

Cleveland Clinic Laboratories returns slides & blocks via FedEx.

Expedited return services are available if a FedEx/UPS account is provided by the requester.

IMPORTANT

Incomplete or incorrectly labeled submissions may delay processing and diagnosis.

Retain a copy of the requisition for your records.

Please complete a Pathology Specimen Manifest and include it with the shipment.

A completed requisition is required for each specimen submitted to Cleveland Clinic Laboratories.

Specimen Collection & Transport

Specimens are only acceptable for processing when they are collected and submitted to our laboratories appropriately.

If you have additional questions regarding the type of container to use for a particular test or specimen, refer to the specimen requirements on the Test Directory or contact Laboratory Customer Service.

Ordering Requirements

To ensure high-quality patient care and efficient use of resources, follow these subspecialty-specific submission guidelines:

Subspecialty-Specific Requirements

Head/Neck/Oral/Thyroid:
• Exclude thyroidectomies greater than 20 H&E slides/blocks

Liver:
• Random/Medical Liver Biopsy:
– Clinical notes and lab test results
• Liver Lesions:
– Imaging report(s)
• Transplant Liver Biopsy:
– Date of transplant
– Clinical notes and lab test results

Cytology:
• Block or 10 unstained slides
• Clinical history
• Pathology, imaging, and surgical pathology reports
• Verification of cytologist/pathologist screening
• Note: Unscreened cases will not be accepted

Medical Kidney:
• Random/Native Medical Kidney Biopsy:
– Clinical notes and lab test results
• Kidney Transplant Biopsy:
– Date of transplant
– Clinical notes and lab test results

Ortho/Bone:
• Strongly recommend imaging and/or imaging report

Pulmonary:
• Neoplastic pathology:
– Clinical notes from oncologist/pulmonologist and pre-biopsy CT report
• Non-neoplastic disease:
– Clinical notes from pulmonologist and chest CT report

Required: Representative block(s) and/or 10 unstained slides for:
• Bone Marrow/Hematopathology
• Lymphoma
• Neuropathology

For all other subspecialties, recommend representative block(s) and/or 10 unstained slides.

Pathology Consultation Kits contain all the necessary supplies to send consults directly to Cleveland Clinic Laboratories.

Each kit includes a range of specimen collection and transport materials as well as a prepaid shipping label for added convenience.

Easy to Use

Sending specimens to our labs with our kits is easy!

Once the specimen has been collected and prepared for transport, simply place the kit in the provided shipping envelope, affix the return label to the outside of the packaging, and arrange for a package pick-up or drop-off.
